Reliance Standard Accidental Death & Dismemberment

This program is sponsored by the University of North Carolina at Chapel Hill. The plan administrator is Reliance Standard Life Insurance Company.

This insurance program provides a benefit if you suffer a loss as the result of a covered accident while you are insured under the plan. It also pays a benefit if you suffer certain disabling injuries while covered. The coverage is effective 24 hours a day, 365 days a year. It includes accidents on or off the job, while traveling by car, plane, train, boat or any other public or private form of transportation.

All permanent part-time (20-29 hours per week) and full-time (30-40 hours per week) employees can elect AD&D coverage. You can elect coverage for your spouse or domestic partner and dependent children up to age 19 years (or 26 years if a full-time student). Your spouse/domestic partner's coverage level cannot exceed your coverage level under the plan.

Premiums: Premiums for this plan are paid solely through employee contribution as an after-tax payroll deduction. Premiums are determined by the amont of insurance you choose for yourself and your spouse/domestic partner and dependent children:

If you are paid on a monthly basis, the full monthly contribution is deducted from each paycheck. If you are paid on a biweekly basis, half of your monthly contribution is deducted from the first two paychecks of the month.

You must enroll within 30 days of your hire date or a qualifying family status change, or during annual enrollment.
