Assurant Dental Insurance

Description

Assurant Dental, previously known as Fortis, is the University's dental carrier. The plan offers comprehensive coverage for preventive, basic and major services. It provides an alternative to the NCFlex dental program. Premiums are paid one month in advance of coverage and are deducted on an after-tax basis.


Eligibility

All permanent part-time (20-29 hours per week) and full-time (30 or more hours per week) employees can elect Assurant coverage for themselves, their spouse/domestic partner and their unmarried dependent children up to age 19 (or up to age 25 if a full-time student).


Cost

Premiums: Premiums for this plan are paid solely through employee contributions on an after-tax payroll deduction.

Deductibles: There is a $50 per person per calendar year deductible for basic and major services and a $50 per person per calendar year deductible for orthodontics.

Annual Maximum: There is a $1,250 annual maximum benefit per covered member. There is a maximum lifetime orthodontic benefit of $1,500 per covered dependent child up to age 19.


Enrollment

You must enroll within 30 days of your hire date, a qualifying family status change or during annual enrollment. You may be subject to certain waiting periods if you are a late enrollee.


Benefits

100% coverage for preventive services with no deductibles

80% coverage for basic services with deductible

50% coverage for major and orthodontic services with deductible

Waiting Periods

Applicants applying for insurance within 30 days of becoming eligible must wait six months for major services to be covered and 12 months for orthodontic services to be covered.

Applicants applying for insurance more than 30 days after becoming eligible will be subject to a six-month waiting period for basic services (restorative), a 12-month waiting period for basic services (non-restorative), and a 24-month waiting period for major and orthodontic services.
