| Description
Long-term care is extended care you would receive at home, in an assisted living or nursing facility, adult day care or hospice.
|
|

| Eligibility
Actively-at-work benefit eligible employees and retirees of the State Health Plan, their spouses, parents, parents-in-law, grandparents and grandparents-in-law, children age 18 and older and their spouses are eligible to enroll.
|

| Cost
Your insurance premium will be determined based on the policy and benefit level your elect. Premiums are paid through after-tax payroll deductions.

Benefits
Long term care insurance will pay for full-time medical or support services if you should suffer a debilitating medical condition. Examples of such conditions include strokes, Alzheimer's Disease or prolonged serious illnesses.
You can learn details about the benefits at Prudential's Web site. When you access Prudential's Web site, you will need to enter certain information:
- The first page asks for a Group Name. In the text box, type "stateplan" (without the quotation marks) and click on "Continue."
- The next page asks for a password. In the text box, type "stateplanltc" (without the quotation marks). In the drop-down box, select "North Carolina" and click on "Login."
