This program is sponsored by the State of North Carolina as part of the NCFlex benefit program. A.C. Newman & Company on behalf of Gerber Life Insurance Company is the plan administrator for the State.


Description

This insurance program provides a benefit if you suffer a loss as the result of a covered accident while you are insured under the plan. It also pays a benefit if you suffer certain disabling injuries while covered. The coverage is effective 24 hours a day, 365 days a year. It includes accidents on or off the job, while traveling by car, plane, train, boat or any other public or private form of transportation.


Eligibility
All permanent part-time (20-29 hours per week) and full-time (30 or more hours per week) employees can elect coverage. Dependent coverage is not available under this program.

Cost

Premiums for this plan are paid solely through employee contribution as a pre-tax payroll deduction. Premiums are determined by the amount of insurance you choose for yourself and your spouse and/or dependent children:

If you elect family coverage and a family member suffers a loss, the benefit paid is a percentage of the amount paid if you suffered a loss. Refer to the NCFlex Benefits Highlights for additional information.

If you are paid on a monthly basis, the full monthly contribution is deducted from each paycheck. If you are paid on a biweekly basis, half of your monthly contribution is deducted from the first two paychecks of the month.


Enrollment

You must enroll within 30 days of your hire date or a qualifying family status change, or during annual enrollment.
