United Concordia Dental Insurance

This program is sponsored by the State of North Carolina as part of the NCFlex Benefit Program. United Concordia is the plan administrator for the State.


Description

The plan offers two options for comprehensive coverage. The high option provides coverage for diagnostic and preventive services, as well as basic, major, and orthodontic services. The low option only provides coverage for  diagnostic and preventive, as well as basic services. The low plan does not pay any benefits for major services or orthodontic services. Both are traditional plans that pay a percentage of the usual and customary charges.


Eligibility

All permanent part-time (20-29 hours per week) and full-time (30 or more hours per week) employees can elect coverage for themselves, their spouse and their unmarried dependent children up to age 19 (or up to age 26 if a full-time student).


Cost

Premiums for this plan are paid solely through employee contribution as a pre-tax payroll deduction.

If you are paid on a monthly basis, the full monthly contribution is deducted from each paycheck. If you are paid on a biweekly basis, half of your monthly contribution is deducted from the first two paychecks of the month.


Enrollment

You must enroll within 30 days of your hire date or a qualifying family status change, or during annual enrollment. You may be subject to certain waiting periods if you are a late enrollee.


Benefits

High Option Plan

  • Includes orthodontia for dependent children under 19
  • $50 deductible per individual for Type II and III services; $150 family aggregate
  • 100% coverage for all diagnostic and preventive services
  • 80% coverage for all basic services except periodontics
  • 50% coverage for all major services including periodontics
  • 50% coverage for orthodontics (for dependent children under age 19)
  • $1,250 annual maximum per person for all services other than orthodontic services (orthodontic services have a lifetime maximum of $1,500)

Low Option Plan

  • Does not include orthdontia coverage for major service coverage
  • $25 deductible; $75 family aggregate
  • 100% coverage for all diagnostic and preventive services
  • 50 to 80% coverage for fillings and simple extractions
  • $1,000 annual maximum per person for all services

Waiting Periods

If you decide to make changes to your dental benefit election or add coverage for the first time, you or your dependents may be subject to waiting periods. The benefit waiting period refers to the amount of time the employee or dependent must be covered by the plan or a qualified after-tax plan before specified benefits are payable. The plan will not pay for (and covered dental services do not include) charges incurred by the insured individual or dependent before the completion of the benefit waiting period.

NOTE: Effective January 1, 2008 United Concordia is the plan administrator for the State.  Prior to January 1, 2008 the plan administrator was National Pacific Dental.  For claim questions or reimbursement prior to January 1, 2008, please call national Pacific at 800-252-0972.
