Non-Salaried to Salaried Fixed Term Appointments
If a faculty member holds a non-salaried fixed term primary appointment and an offer is extended for a salaried fixed term appointment, the following documents will be required:
- End of Employment EPAWeb action to end non-salaried position
- New Employment EPAWeb action for new salaried position
- Recommendation letter from the dean
- Updated curriculum vita
- Two letters of recommendation
- Updated AP2 to reflect the new employment information
Documents normally required for a salaried fixed term initial appointment will not be required if they were included as part of the non-salaried fixed term "Standard Order" paperwork. The faculty member may choose to revise tax cards for the salaried appointment.
The following documents, if not completed for the non-salaried appointment, are required:

I-9 forwarded to EEV Coordinators in HR Employment, Classification & Compensation after EEV verification is completed (Write approved EEV number on top of I-9 form.)
- Tax Cards (send directly to Payroll)

Payroll Direct Deposit Form (send directly to Payroll)
