NCFlex – MetLife

This program is sponsored by the State of North Carolina as part of the NCFlex Benefit Program. MetLife is the plan administrator for the State. The plan offers two options for comprehensive coverage. The high option provides coverage for diagnostic and preventive services, as well as basic, major, and orthodontic services for dependent children up to age 19. The low option only provides coverage for diagnostic and preventive, as well as basic services. The low plan does not pay any benefits for major services or orthodontic services. Both are traditional plans that pay a percentage of the usual and customary charges.

All permanent part-time (20-29 hours per week) and full-time (30 or more hours per week) employees can elect coverage for themselves, their spouse and their unmarried dependent children up to age 26.

To print a card, visit the MyBenefits website at www.metlife.com/mybenefits (enter ‘NCFlex’ as the company name). You can create your own unique User ID and password. Click on the Register Now button to provide your first name, last name, date of birth, SSN and e-mail address.

MetLife – Domestic Partner Plan

This program is sponsored by the University of North Carolina at Chapel Hill. MetLife is the plan administrator. The plan offers two options for comprehensive coverage. The high option provides coverage for diagnostic and preventive services, as well as basic, major, and orthodontic services for dependent children up to age 19. The low option only provides coverage for diagnostic and preventive, as well as basic services. The low plan does not pay any benefits for major services or orthodontic services. Both are traditional plans that pay a percentage of the usual and customary charges.

This plan provides coverage for an employee’s domestic partner. An employee MUST be enrolled under the NCFlex MetLife Dental Plan to select this plan for their domestic partner.
