EPA Non-Faculty Initial Appointment Required Documents

CONTENTS

Introduction

The following list of documents constitutes what we refer to as the "Standard Order" of paperwork which must be submitted for initial EPA non-faculty appointments.

Please scan and attach approved copies of all required forms and documents to the New Employment EPAWeb action:

  • Recommendation letter
  • AP2
  • Curriculum vitae
  • AP2a
  • AP2b
  • EEO approval
  • Appointment/contract letter
  • EPA NF Position Approval Memorandum

Recommendation Letter

Recommendation letter from the Dean or Division to the Provost or from the Chair endorsed by the Dean.  This is a requirement for all EPA non-faculty appointments.


AP2

An AP-2 form is to be completed for a new appointee who is Exempt for the Personnel Act (EPA).  Please verify that information is correct.


Curriculum Vitae (CV)

Provide a full curriculum vitae detailing education, previous experience, publications, and presentations.

Important Note: CVs should not include age, date of birth, marital status, or social security number (SSN). These items are not relevant and should always be omitted from the CV.


#Copies of Documents

Only one copy of the above documents should be forwarded to the Provost office with the following (if documents were not attached to the EPAWeb action):

  • Copy of New Employment EPAWeb action (or Advanced Transmittal Form)
  • Non-Faculty Appointee Certifications and Conditions of Employment (Form AP2a
  • EEO Approval
  • Copy of appointment/contract letter (prepared by the unit)
  • Copy of EPA NF Position Approval Memorandum

Disposition of the following documents:

  • Tax Cards (send directly to Payroll)
  • Payroll Direct Deposit Form (send to Payroll)
  • OP1 (keep in dept office)
  • AP2b (send original to Public Safety Background Investigator)
  • I-9 forwarded to EEV Coordinators in HR Workforce Planning & Compensation after EEV verification is completed (Write approved EEV number on top of I-9 form)
