Name and/or Address Changes
If your home address changes while you are employed with the University, it is your responsibility to contact the University and your benefits vendors about the change.
You must change your home address and phone number in the online campus directory. This change will be processed by the relevant University offices, including Payroll and Human Resources. The campus directory is available here. You will need your Onyen and associated password to sign-on.
The University will use your address updated in the campus directory and send this data electronically to the following benefit providers: AIG Retirement, Fidelity, Teachers and State Employees' Retirement System (TSERS), Prudential (State 401(k) and 457(b) plans) and all NCFlex providers, including: United Concordia Dental, Superior Vision, Aon Consulting for Healthcare/Dependent Care spending accounts, ING, UNUM and American Heritage Cancer plan.
It is essential that you also update other benefits providers that we do not send electronic feeds to with your new home address and/or telephone information. The following providers do not receive electronic address changes from the University; therefore, you will need to update them with your new home address or telephone number to ensure that you continue to receive important information directly from these providers regarding your benefits. Address change forms for these providers can be downloaded here and returned to Benefits Services, CB# 1045.
If you are covered by a University benefits provider that is not included on this page, a change of address notice is not required.
To change your address with an Optional Retirement Program or Supplemental Retirement provider not listed above, you must contact them directly using the information below.
Name Change
If you get married, divorced, or otherwise have a legal change in your name, provide this information to your Human Resource Facilitator, who will initiate and submit the correct paperwork to have your name changed through the payroll system.
