OPTION #1: SEND AWARD LETTER AND GIFT CERTIFICATE:

If you have already informed the recipient of the award, select below the award that the recipient has chosen. Employee Services will send a congratulatory award letter along with the gift certificate to the recipient.

OPTION #2: SEND AWARD LETTER ONLY:

If you want to suprise the recipient with an award letter first, complete the information below. Employee Services will send the congratulatory letter to the department's HR Facilitator for delivery to the recipient. Instructions to the recipient for selecting the gift certificate will be included with the letter.

Recipient information is published in the first available issue of the University Gazette. If you need to delay the publish date to coincide with your announcement of the award to the recipient, please indicate the earliest date this information may be published. (The date you list does not have to be an actual Gazette publication date.)
