Wellness Checks

Register or walk-in for a free wellness check. In just 15 minutes you will have a comprehensive wellness review of your physical health including results for cholesterol, blood glucose/A1c, blood pressure, body mass index and waist circumference. (PDF registration instructions)
Wellness check registration
Knowing your numbers will let you know if you’re at risk for developing or have an undiagnosed chronic condition.
- You must be a NC State Health Plan member for the 2023 benefit year to participate.
- Appointments are preferred but not required.
- Walk-in appointments are on a first-available basis.
- Screenings appointment slots are limited.
- Your results will NOT be shared with your employer or the NC State Health Plan.
- Participants with abnormal results should contact their primary care provider for follow-up.
- Your 2023 NC State Health Plan insurance card
- Photo ID (UNC One Card accepted)
- Advance wellness check registration is open through Friday, March 10.
- The wellness checks will be performed Wednesday, March 15.
- Screenings will begin at 9 a.m. and continue throughout the Expo.
