Dear HR Officers,

With the upcoming spring semester, employees seeking COVID-19-related accommodations, including a full- or part-time teleworking schedule, must complete and submit updated documents to the Equal Opportunity and Compliance Office (EOC). These forms include:

An updated COVID-19 Accommodation Request Form (filled out by the employee)
An updated COVID-19 Medical Documentation Form (filled out by the employee’s healthcare provider)

It’s important for the employee to ensure both forms are completed before submitting them to EOC. Incomplete forms will be returned to the employee and Accommodation Request Forms submitted without a Medical Documentation Form will not be reviewed. Completed requests should be sent to and must be submitted by:

Dec. 10, 2021: Instructors requesting remote teaching as an accommodation
Dec. 13, 2021: Staff request remote work as an accommodation

Provided that all forms are completed and submitted, instructors will receive a notice of decision from EOC by Dec. 23, 2021, while staff will receive notice by Jan. 4, 2022.
