With the upcoming spring semester, employees seeking COVID-19-related accommodations, including a full- or part-time teleworking schedule, must complete and submit updated documents to the Equal Opportunity and Compliance Office (EOC). These forms include:
It’s important for the employee to ensure both forms are completed before submitting them to EOC. Incomplete forms will be returned to the employee and Accommodation Request Forms submitted without a Medical Documentation Form will not be reviewed. Completed requests should be sent to firstname.lastname@example.org and must be submitted by:
Dec. 10, 2021: Instructors requesting remote teaching as an accommodation
Dec. 13, 2021: Staff request remote work as an accommodation
Provided that all forms are completed and submitted, instructors will receive a notice of decision from EOC by Dec. 23, 2021, while staff will receive notice by Jan. 4, 2022.
