How to submit forms

Leave

Starting Monday, Nov. 14, 2022, employees will be able to submit leave attachments online through ConnectCarolina.

Log in to ConnectCarolina using your Onyen and password then go to: SelfService > My Benefits > Leave > Submit a Leave Document.

Employees will be able to submit attachments including:

  • Leave request forms
  • Medical certifications
  • Return-to-work forms

Leave forms may be emailed to leave@unc.edu for processing through Friday, Nov. 11, 2022. Beginning Monday, Nov. 14, 2022, leave attachments will no longer be accepted via email.

If an employee is not able to submit a leave attachment via ConnectCarolina, the employee’s departmental HR Representative can submit the attachment in ConnectCarolina on the employee’s behalf. HR Reps will not have access to view an employee’s leave documents in ConnectCarolina once the leave attachment eForm has been executed.

Employees can view uploaded leave attachments in ConnectCarolina at SelfService > My Benefits > Leave > View My Leave Forms.

Benefit forms should be emailed to benefits@unc.edu for processing.

Forms

Mandatory and supplemental retirement elections are managed electronically through ConnectCarolina. To enroll, terminate or submit changes to your mandatory and/or supplemental retirement plans:

  1. Log in to ConnectCarolina
  2. Choose Self Service from the drop-down menu at the top
  3. Choose the My Benefits tile
  4. Choose Benefits Enrollment on the left
  5. Select Mandatory Retirement Plan OR Supplemental Retirement Plans

If you want to retire from TSERS or the ORP, please send an email to benefits@unc.edu indicating your name and PID. You can request that a Benefits Consultant reach out to you.

Leave Forms

Below are links to the leave forms with the type of attachment that should be used to submit the form in ConnectCarolina by going to SelfService > My Benefits > Leave > Submit a Leave Document.

Note: Employees can use ConnectCarolina to submit types of leave attachments other than the ones listed below (for example, birth or adoption documentation). For a comprehensive list of leave document attachment types, please review the “File Attachment Description” section of user guides linked above.
