The State Health Plan dependent eligibility audit began on May 22, 2017 and concludes on July 31, 2017.  Plan participants who cover dependents will receive a letter from the State Health Plan by the end of this month.  The purpose of the audit is to ensure that all dependents currently enrolled in the State Health Plan are eligible to receive coverage under the Plan.  This particular audit applies to dependent eligibility for coverage under the State Health Plan, only.

Who Needs to Take Action

State Health Plan members who currently cover dependents (spouse and/or children) under the Plan must verify each dependent’s eligibility to participate in the Plan. Under the audit process, members are required to respond and submit the necessary documentation for each covered dependent no later than July 31, 2017. For a list of acceptable documentation, click hereFailure to respond and produce the required documentation will result in termination of the dependent’s State Health Plan coverage effective August 1, 2017.

How to Provide Documentation

Plan participants may submit documentation through ConnectCarolina, fax or email.

To submit through ConnectCarolina:

  1. Sign in to ConnectCarolina and click on Self Service and then on Enroll or Change Benefits.
  2. Once you are logged into eEnroll a message box will appear: “Dependent Verification Documentation Request”
  3. Click GET STARTED.
  4. Follow the prompts to upload the required documentation within the Document Center. You may provide a scanned copy of the document or take a photo with your smart phone and upload it directly to the Document Center.
  5. If the documentation you provide cannot be verified, you will receive notification.
  6. Check the status of your document submission periodically to ensure the process is complete!

Submit by fax or email: As an alternative, you can fax the required documents to 866-742-6444 or email the documents to SHPDependentAudit@benefitfocus.com.  Your full name, dependent’s full name, State Health Plan ID number located on your Plan ID card and the name of your employing unit (UNC Chapel Hill) are required on both the email and fax in order for the document to be successfully approved and uploaded to your record. Mailed, hard copy documents will not be accepted, nor will documents submitted to OHR-Benefits.  Documents must be submitted by one of the three methods provided above.

PLEASE NOTE: If you are submitting any documentation that contains Social Security number data, you should black out the number. Social Security numbers are not required for you or your dependent(s) as part of the dependent verification process. You may also black out any account information, financial transactions, account balances and any other information that is not pertinent to the process.
